Referrals can be made for people who are over 65 (or younger with chronic illnesses by exception) and who have had their own dogs but are unable to have one any more.
Referrals can be made by professional organisations involved in health and social care, for example, your GP, the NHS Trust, Adult Social Services, Occupational Health etc.
